Care Advantage Inc. in Colonial Heights, VA seeks a high-energy Care Coordinator to connect clients with compassionate caregivers and manage scheduling in a fast-paced environment.

You’ll pair clients with caregivers based on health needs and preferences, coordinate with families and staff, and ensure accurate, up-to-date records while maintaining excellent communication and a client-first mindset.

Qualifications

• High School Diploma or GED required.
• 2+ years of relevant work experience.
• Scheduling, medical, or phone experience is a plus.
• Computer literacy with scheduling systems.
• Strong organizational, attention to detail, and communication skills.

Responsibilities

• Pair caregivers with clients by health needs, personality fit, skill level, and preferences.
• Act as the liaison between clients, families, and field staff; build trust and ensure smooth communication.
• Maintain accurate, up-to-date schedules in the system and manage multiple moving parts.
• Pivot quickly during emergencies or last-minute changes to keep clients cared for and caregivers supported.
• Monitor field staff activity, attendance and reliability; provide feedback to ensure accountability.
• Assist onboarding of new clients; guide them through the process and set a caring tone.
