Overview
The Care Coordinator RN (CC RN) is responsible for assessing transitional care needs, coordinating care across the continuum, and engaging with patients and families to ensure care needs are met. The CC RN plans effectively to meet the patient's needs, manage the length of stay, and promote efficient utilization of resources. This role involves care facilitation, care progression, and transitional care planning functions.
Key Responsibilities
- Assess psychosocial and functional status, plan transitional care, facilitate patient/family conferences, participate in interdisciplinary rounds, and provide patient/family education.
- Collaborate with utilization review nurses, physicians, and the interdisciplinary care team to assess medical, psychosocial, and social determinant needs, and facilitate continued care.
- Ensure patient progress towards discharge goals and address barriers.
- Consult with relevant disciplines to identify and resolve delays, and expedite care and discharge processes.
Core Responsibilities
- Conduct assessments of patients' chronic disease management and psychosocial risks.
- Partner with the team to evaluate insurance coverage and requirements.
- Collaborate with patients, families, and care teams to develop and support care and discharge plans.
- Manage all aspects of discharge planning, including developing individualized plans, identifying barriers, and coordinating post-acute care referrals.
- Document assessments, care plans, and interactions accurately and timely in the medical record.
- Support professional development and serve as a preceptor or mentor as appropriate.
Qualifications
- Associate's Degree in Nursing from an accredited institution; Georgia RN License required.
- Minimum 1 year of nursing experience in an acute care setting.
- Excellent communication skills; ability to work under stress and adapt to change.
- Knowledge of relevant regulations and community resources.
- Licenses and certifications: RN license (single state and multi-state compact), BLS certification (including instructor and provisional if applicable).
