Overview
The Care Coordinator RN (CC RN) is responsible for assessing transitional care needs, coordinating care across the continuum, and engaging with patients and families to ensure care needs are met. The CC RN plans effectively to meet patient needs, manage length of stay, and promote resource utilization. The role integrates and coordinates care facilitation, progression, and transitional care planning functions.
Responsibilities
- Assess psychosocial and functional status, plan transitional care, facilitate care conferences, participate in rounds, and educate patients and families.
- Collaborate with the utilization review nurse, physicians, and care team to assess medical, psychosocial, and social determinants of health needs, and support goal/outcome attainment.
- Ensure patient progress towards discharge goals and help address barriers.
- Consult with relevant disciplines to identify and resolve delays, and facilitate timely discharge.
Core Responsibilities and Functions
- Assessment: Initiate assessments of chronic disease management, psychosocial risks, and discharge resources based on initial screening.
- Care Progression: Collaborate with physicians and care team to facilitate communication and address delays in care.
- Disposition Planning: Manage discharge planning, develop individualized plans, and coordinate post-acute referrals.
- Documentation: Complete and maintain clear, concise records of assessments, interactions, and care plans.
- Professional Development: Complete required training, support departmental goals, and serve as a preceptor if appropriate.
Qualifications
- Associate's Degree in Nursing and Georgia RN License required.
- Minimum 1 year nursing experience in acute care.
- Excellent communication skills, maturity, self-confidence, and ability to work under stress.
- Knowledge of regulations and community resources.
- RN license (single state and multi-state compact), BLS, and BLS Instructor certifications required.
