Overview
The Care Coordinator RN (CC RN) assesses transitional care needs, coordinates care across the continuum, and engages with patients and families to ensure care needs are met. The role involves planning to meet patient needs, managing length of stay, and promoting resource utilization. It integrates care facilitation, progression, and transitional planning functions.
Key Responsibilities
- Assess psychosocial and functional status, plan transitional care, facilitate conferences, participate in rounds, and educate patients/families.
- Collaborate with utilization review nurses, physicians, and care teams to evaluate medical and psychosocial needs, social determinants, and ongoing care requirements.
- Ensure patient progress towards discharge goals and address barriers.
- Consult with relevant disciplines to identify and resolve delays, expedite care, and facilitate discharge.
Core Responsibilities
- Assess patient’s chronic disease management and psychosocial risks based on initial screening.
- Partner with the team to evaluate insurance coverage and requirements.
- Collaborate with patients, families, physicians, and care teams to support care progression and discharge planning.
- Facilitate communication regarding care progression and address delays in treatment.
- Manage discharge planning, develop individualized plans, and coordinate post-acute care referrals.
- Document assessments, interactions, and care plans clearly and timely in medical records.
- Support professional development and serve as a mentor or preceptor if appropriate.
Qualifications
- Associate's Degree in Nursing and Georgia RN License (Required).
- Minimum 1 year nursing experience in acute care (Required).
- Excellent communication skills, maturity, confidence, and ability to work under stress.
- Knowledge of regulations, community resources, and care transition processes.
- RN license valid in Georgia and multi-state compact, BLS certification (Required).
