Qualifications

• BSN or higher from an accredited program.
• Active and unencumbered RN license.
• 1+ years of case management experience, preferably in healthcare or hospital settings.
• Strong interpersonal and written/verbal communication skills for collaboration with medical professionals, patients, and families.
• Proficiency in electronic health records (EHR) software.

Responsibilities

• Conduct comprehensive patient assessments including medical history, medications, treatment plans and psychosocial needs.
• Collaborate with healthcare providers, patients and families to develop and implement individualized care plans.
• Monitor and evaluate patient progress and adjust care plans as needed; communicate changes to the healthcare team.
• Serve as main point of contact between patient and healthcare providers to ensure seamless communication and care coordination.
• Maintain up-to-date records and case notes in compliance with confidentiality standards.
