Job summary

Main Line Health is seeking a Care Coordinator RN at Riddle Hospital. The role centers on orchestrating cross-continuum care with physicians and families to create a comprehensive plan addressing health promotion and disease management.

You will drive care transitions and partner with the team to improve patient experiences and outcomes. Qualified candidates have 3 years of recent clinical experience, with leadership experience preferred, and an active PA RN license.

Responsibilities

• Coordinate care across physicians, peers, and families to develop a comprehensive plan of care.

• Collaborate to improve patient experiences, outcomes, and system-wide population health.
• Participate in committees and projects to advance evidence-based practice and care coordination.

Experience:

3 years of recent clinical experience required, leadership experience preferred. If ADN, 10+ years of experience required.

Education:

1. Associates Degree OR BSN required upon hire; if Associates, BSN required within 3 years of hire

2. Master's degree preferred

Licensures/Certifications:

Active PA RN license required
