The Care Coordinator is responsible for managing the case coordination for patients assigned to them. This includes ensuring an excellent level of care is provided regarding infusion pharmacy services.
Responsibilities
- Maintain complete, organized, and accurate files for all patients related to coordination of services: case management, nursing services, pharmacy preparation and delivery, and relevant data for overall patient case coordination.
- Input and update data for all patients, from initial notification through verification of benefits, initiation of care, follow-up, completion of services, discharge, and cessation of communications.
- Complete all required reports timely as requested by the Intake Manager, General Manager, or ProHealth.
- Adhere strictly to policies and procedures, ensuring compliance with state guidelines and current knowledge of all relevant areas.
- Identify and resolve any case coordination issues, documenting concerns or deficiencies and reporting them to the Intake Manager.
- Communicate professionally and courteously with internal and external stakeholders involved in patient case coordination, including hospital discharge planners, home health staff, physicians, case managers, and others.
- Participate in orientation and training of new staff regarding referral processes, documentation, cross-training, and work duties.
- Verify and, when directed, train health professionals on home infusion care, including educational assessments, presentations, and meetings.
- Engage in QA&I activities, monitor outcome indicators, and contribute to the Beyond Expectations program.
- Maintain patient confidentiality at all times.
- Participate in continuing education and in-service training as required.
- Perform other duties as assigned.
Qualifications
- High School diploma or GED
- At least one year of healthcare, contact center, or medical experience in home health or infusion
- Two years of experience in home infusion with strong clinical knowledge is preferred
- Proficiency in Microsoft Suite
