Complete Care seeks a Care Coordinator (Rehab) to support patient recovery by delivering provider-directed therapies, preparing patients for sessions, and ensuring care plans stay on track. You will document therapies in the EHR, educate patients on exercises and DME use, and collaborate closely with clinicians to provide a seamless patient experience.

Responsibilities

  • Deliver provider-directed rehabilitation therapies safely and accurately.
  • Prepare patients for therapy and explain expectations.
  • Help patients stay on track with prescribed treatment plans.
  • Document therapies accurately in the electronic medical record (EHR).
  • Educate patients on therapy exercises and proper use of DME.
  • Monitor patient progress and communicate concerns to providers.
  • Verify insurance information and support authorization workflows.
  • Coordinate with providers and teammates to ensure a seamless patient experience.

What You'll Do
  • Deliver provider-directed rehabilitation therapies safely and accurately.
  • Prepare patients for therapy and explain what to expect.

  • Monday: 7:30 AM – 11:00 AM & 2:30 PM – 6:00 PM
  • Tuesday: 9:30 AM-1:00 PM & 3:30 PM– 7:00 PM
  • Thursday: 9:30 AM-1:00 PM & 3:30 PM– 7:00 PM
  • Friday: 6:30 AM – 7:30 PM
  • Saturday: 8:30 AM-5:30 AM
