Job summary

Complete Care is seeking a Care Coordinator (Rehab) to support patient recovery by delivering provider-directed therapies and ensuring accurate, efficient care. You will prepare patients, explain treatment plans, and document activities in the EHR.

The role emphasizes compassionate, organized care with collaboration across clinicians and staff to create a positive patient experience.

  • Educate patients on therapy exercises and the proper use of Durable Medical Equipment (DME).
  • Monitor patient progress and communicate concerns to providers when appropriate.

What You'll Do
  • Deliver provider-directed rehabilitation therapies safely and accurately.
  • Prepare patients for therapy and explain what to expect.
  • Help patients stay on track with their prescribed treatment plans.
  • Document therapies accurately in the electronic medical record (EHR).
  • Educate patients on therapy exercises and the proper use of Durable Medical Equipment (DME).

  • Verify insurance information and support authorization workflows.
  • Coordinate closely with providers and teammates to deliver a seamless patient experience.
  • Maintain an organized treatment area while supporting efficient daily clinic operations.

  • Tuesday: 9:30 AM-1:00 PM & 3:30 PM- 7:00 PM
  • Thursday: 9:30 AM-1:00 PM & 3:30 PM- 7:00 PM
  • Friday: 7:30 AM - 2:00 PM
