Job summary

Complete Care, LLC in Melbourne, FL is seeking a Care Coordinator (Rehab) to support patient recovery by delivering provider-directed therapies, preparing patients for sessions, and documenting care in the EHR. You will coordinate with clinicians to ensure seamless patient pathways and maintain an organized clinic environment.

You will educate patients on exercises and DME, monitor progress, verify insurance, and help sustain a patient-centered rehabilitation program.

Qualifications

• Experience as Medical Assistant, Physical Therapy Aide, Rehabilitation Technician, Chiropractic Assistant, Patient Care, Customer Service or Healthcare.
• Ability to follow clinical procedures and patient care protocols.
• Strong communication and organizational skills.

Responsibilities

• Deliver provider-directed rehabilitation therapies safely and accurately.
• Prepare patients for therapy and explain what to expect.
