Complete Care seeks a Care Coordinator (Rehab) to support patient recovery through therapy delivery, patient preparation, and precise documentation in the EHR. The role emphasizes compassionate, organized care and collaboration with providers in a fast-paced clinic.

The ideal candidate will help patients follow treatment plans, educate on exercises and DME, and contribute to a positive clinic experience while maintaining high standards of patient safety and service.

• Educate patients on therapy exercises and proper use of DME.
• Monitor patient progress and communicate concerns to providers when appropriate.
• Verify insurance information and support authorization workflows.
• Coordinate with providers and teammates for a seamless patient experience.
• Maintain an organized treatment area and support clinic operations.
• Interact professionally with patients, fostering empathy and care.
• Identify opportunities to improve workflows and patient care.

• Tuesday: 12:00 PM- 7:30 PM
