Job summary

Kern Medical Outpatient Health - Reach & Grow LLC Clinics in Bakersfield, CA is seeking a Care Coordinator (PCMH) for full-time, on-site employment. Bilingual preferred.

The role coordinates care across the health care continuum, serving as the link between patients and primary care physicians to promote seamless, high-quality care. Responsibilities include managing caseload by acuity, coordinating referrals and services, documenting in EMR, and preparing Health Action Plans within 90 days.

Qualifications

• High School Diploma, G.E.D. or equivalent and completion of an accredited Medical Assistant program.
• MA Diploma (State Cert if MA-C) and BLS through AHA required.
• Current AHA BLS certification.

Responsibilities

• Manage assigned case load based on patient acuity.
• Maintain contact with patients per program schedule.
• Coordinate with providers on refill requests.
• Collaborate with care team to arrange services.
• Schedule upcoming appointments and manage records.
• Document care coordination in EMR.
