Job summary

Kern Medical in Bakersfield is seeking a Care Coordinator for the PCMH program in Reach and Grow LLC Clinics. This full-time, on-site role coordinates patient care across the health care continuum and connects community patients with their primary care providers.

The ideal candidate has a Medical Assistant diploma/certification and BLS through AHA. Bilingual is preferred. Compensation ranges from $25.2641 to $33.9528 hourly with potential incentives and 401(k) contributions.

Qualifications

• High School Diploma, G.E.D. or equivalent.
• Successful completion of an accredited Medical Assistant program.

Responsibilities

• Coordinate care across the health care continuum.
• Serve as the link between community patients and the primary care physician.
• Manage patient contacts and follow-up within program timelines.
• Collaborate with care team on referrals, documents and orders.
• Document care coordination in EMR and maintain program census.
