• Strong communication with patients, families, physicians, and staff.
• Ability to manage multiple priorities in home health setting.

Responsibilities

• Coordinate patient referrals, admissions, intake, transfers, recertifications, resumptions of care, and discharges while keeping care processes organized.
• Communicate professionally with patients, families, physicians, referral sources, and clinical staff to provide responsive service.
• Obtain, track, and maintain physician orders and required documentation; review charts for missing information and ensure timely completion.
• Serve as liaison between clinical team, patients, families, physicians, and other healthcare providers to maintain patient-centered care.
• Apply healthcare experience in home health or similar settings; support clinical director and team with daily coordination and administrative tasks.
• Assist with meeting preparation and documentation; work independently yet collaborate with the care team.
• Maintain strong computer skills and EMR/EHR usage; uphold HIPAA and privacy requirements.
