Job summary

WakeMed Health & Hospitals is seeking a Case Management Representative with Population Health to help patients and caregivers navigate the healthcare journey by connecting them with resources, services, and support.

You will collaborate with providers, clinical teams, and community partners to identify barriers to care, coordinate follow-up, and promote better health outcomes. This role emphasizes patient-centered advocacy and interdisciplinary teamwork in a flexible hybrid environment.

Qualifications

• Two (2) years of clinical acute care, healthcare acute care, or related healthcare experience required.
• Must live within traveling distance to the Raleigh Campus of WakeMed.
• Strong communication and relationship-building skills with the ability to connect with patients, caregivers, providers, and clinical teams.
• A patient-centered mindset with a passion for advocacy and helping individuals overcome barriers to care.
• Strong organizational and follow-up skills with the ability to coordinate multiple patient needs and priorities.
• A collaborative approach and willingness to work as part of an interdisciplinary healthcare team.
• Resourceful problem-solving skills and the ability to help patients identify practical solutions to healthcare challenges.
• Commitment to providing compassionate, respectful, and high-quality service.

Responsibilities

• Conduct patient outreach and intake interviews to gather information and identify individual needs, concerns, and barriers to care.
• Help patients and caregivers navigate healthcare services and community resources.
• Connect patients with appropriate resources to support access to care and improved health outcomes.
• Collaborate with providers and clinical team members to coordinate care and ensure timely follow-up.
• Support patients in developing confidence in managing their health through education, advocacy, empowerment, and access to resources.
• Help strengthen positive relationships between patients and their primary care providers.
• Facilitate efficient delivery of services across the continuum of care.
• Serve as an advocate for patients and help address challenges that may impact their ability to receive needed care.
• Provide administrative support to the Population Health and care management teams as needed.
