Qualifications

• Minimum 2 years of clinical acute care experience.
• Must live within traveling distance to the Raleigh Campus of WakeMed.
• Strong communication and relationship-building skills with patients, caregivers, providers, and clinical teams.
• Patient-centered mindset with a passion for advocacy and helping individuals overcome barriers to care.
• Strong organizational and follow-up skills to coordinate multiple patient needs.
• Collaborative approach and willingness to work as part of an interdisciplinary healthcare team.
• Resourceful problem-solving skills to help patients identify practical solutions.
• Commitment to providing compassionate, respectful, and high-quality service.

Responsibilities

• Conduct patient outreach and intake interviews to gather information and identify needs.
• Help patients and caregivers navigate healthcare services and community resources.
• Connect patients with resources to support access to care and outcomes.
• Coordinate care with providers and clinical teams for timely follow-up.
• Educate, advocate, empower patients to manage their health.
• Support relationships between patients and primary care providers.
• Facilitate delivery of services across the care continuum.
• Advocate for patients and address barriers to receive needed care.
• Provide administrative support to Population Health and care management teams.
