MLK Community Healthcare seeks an ED Care Coordinator for full-time night shifts (11pm-7:30am) in Los Angeles, CA. The role coordinates patient transitions of care, partnering with care managers, physicians, and families to ensure timely discharge planning.

You will advocate for patients, arrange post-acute referrals, schedule follow-ups, and monitor care barriers. Strong communication and organizational skills are essential; bilingual Spanish helpful but not required.

Qualifications

• High School Diploma or GED required.
• College degree or vocational training in healthcare preferred.

Responsabilités

• Assists patients through the healthcare system as advocate and navigator.
• Coordinates continuity of patient care with external healthcare organizations and facilities.
• Makes outpatient follow up appointments as requested.
• Coordinates referrals to post-acute facilities (home care, DME, SNF, LTACH, rehabilitation).
• Coordinates post-discharge calls to follow up on labs, instructions, and resources.
• Reports care barriers to care manager and follows transition plans.
• Ensures medications and prescriptions are available prior to discharge.
• Supports data collection, health outcomes reporting and clinical audits.
