About Valley Cities

Valley Cities has recently been selected to lead efforts in Washington State to become a Certified Community Behavioral Health Clinic (CCBHC), reflecting compliance with stringent national standards in behavioral health care. This status enables broader service availability, improved integration of mental health and substance use treatment, enhanced crisis intervention capabilities, and ensures services are accessible to all, regardless of financial circumstances.

Job Summary:

Be the connective thread for clients navigating behavioral health, medical, and social service systems. As a Care Coordinator, you'll deliver Targeted Case Management including assessing needs, building care plans, and making sure nothing falls through the cracks with a critical seat on the treatment team.

What You'll Do (Essential Functions)

  • Conduct comprehensive assessments and build person-centered care plans with clients and their support systems

  • Coordinate care across behavioral health, primary care, and community-based organizations including Designated Collaborating Organizations (DCOs)

  • Support smooth transitions after hospitalization, ED visits, or residential treatment, with timely follow-up contact

  • Advocate for clients and help them overcome barriers to accessing care

  • Maintain accurate documentation in line with CCBHC and Medicaid requirements

What You'll Bring (Required)

  • BA/BS in a human services field

  • Ability to obtain Washington State Agency Affiliated Counselor (AAC) registration within 2 weeks of hire

Nice to Have (Preferred)

  • Experience in care coordination, case management, or health home services

  • Familiarity with DCOs, managed care organizations, or community-based social service systems

Why Valley Cities

  • Carry a manageable caseload, not an unbounded book of clients.

  • Partner daily with clinicians, RNs, and prescribers as a valued member of an integrated care team.

  • Build a strong foundation for a future in care coordination leadership, case management leadership, or clinical licensure.

  • Help define what CCBHC care coordination looks like in Washington State: the practices you help establish now will set the standard for the clinics that follow.
