Responsibilities

• Manage Care Transitions & Integration requests and referrals.
• Coordinate authorization procurement for third party payers with clinical documentation.
• Facilitate care coordination for complex patient needs and assist with procedures and placement.
• Ensure transportation arrangements for appointments.
• Follow up test results and document in the EMR for physician access.
• Communicate effectively with clinical staff, patients and vendors.
