Access Community Health Network is seeking a Care Coordinator (RN or Licensed Social Worker/ Counselor) to support patients navigating care transitions and empower self-management. The role leads intake, care planning, and proactive outreach across care settings.
The position requires an IL license (LCPC/LPC/LCSW/LSW or RN with IA, IL requirements), and a minimum of two years in a clinical setting for social work or one year for nursing; bilingual Spanish is a plus with EMR and MS Office

The Care Coordinator is a Registered Nurse, Licensed Social Worker or Counselor who provides care coordination, social support services and outreach services to assist patients in navigating health care transitions, and support patient empowerment through health education, advocacy and coaching.
Bachelor’s degree required, Master’s degree preferred
Current, unrestricted Illinois license: LCPC, LPC, LCSW or LSW required
Minimum two (2) years’ experience in a clinical setting required.
Associate’s degree required; Bachelor’s degree preferred
Registered Nurse (RN); current unrestricted licensure in Illinois, required
Minimum of one (1) year of general nursing experience in a clinical setting is required.
