Job summary

Presbyterian Healthcare Services is hiring a Care Coordinator IV in Albuquerque, NM. The role focuses on coordinating care across medical, behavioral, and social services using an interdisciplinary team approach to ensure cost-effective, high-quality outcomes.

Responsibilities include comprehensive risk assessments, care planning, and monitoring strategies for members with chronic conditions. Strong MS Word/Excel/Outlook and decision-support skills are needed.

Qualifications

  • Masters degree and 4 years of exp.
  • Bachelor's degree and 8 yrs of exp.
  • Associates degree and 9 years of exp.
  • 12 years of exp may be utilized in lieu of other education and experience reqs.
  • Proficiency in Microsoft Word, Excel and Outlook required.
  • Experience in analyzing trends based on decision support systems.
  • Business management skills to include cost/benefit analysis, negotiation, and cost containment.
  • Knowledge of referral coordination to community & private/public resources.

Responsibilities

  • Facilitates a team approach with the Interdisciplinary Care Plan team to ensure interventions and cost-effective delivery of quality care across the continuum.
  • Collaborates with the Interdisciplinary Care Plan Team including member, caregivers, member's legal representative, physician, care providers, and ancillary support services.
  • Coordinates care of individual clients across medical, behavioral, social, community-based or long-term care services.
  • Conducts in-depth health risk assessment and comprehensive needs assessment (psycho-social, physical, medical, behavioral, environmental, financial).
  • Provides care coordination to members with chronic or complex conditions with intensive interventions and oversight.
  • Implements, coordinates, and monitors strategies to improve health and quality of life outcomes.
