Job summary

Presbyterian Healthcare Services is seeking a Hospice RN in Albuquerque to provide compassionate, comprehensive end-of-life care in a patient- and family-centered model. You will collaborate with an interdisciplinary team to deliver therapeutic, culturally attuned care and to advocate for each patient’s final wishes.

Responsibilities include conducting assessments, following physician orders, and mentoring new staff while maintaining high-quality documentation.

Qualifications

• Associate Degree in Nursing is required.
• Registered Nurse licensure (Compact/Multi) is required.
• Minimum one year in direct patient care or home health.
• Valid driver's license; BLS is required at hire.

Responsibilities

• Provide compassionate end-of-life care with clinical excellence and family support.
• Deliver physical, emotional, and spiritual care per individualized hospice plans.
• Collaborate with LPNs, CNAs, physicians, and partners for seamless care.
• Mentor new hires and clinical students as a trusted resource.
• Maintain accurate clinical documentation in alignment with standards.
• Balance caseloads while meeting productivity and visit targets.
