What to Expect (Job Responsibilities):
- Independently manage a specified caseload of members, creating individualized care plans and coordinating services
- Educate members on benefits such as Medicare and Medicaid, addressing questions and concerns
- Collaborate with physicians, providers, and county financial entities to discuss care plans and member progress
- Conduct telephonic and in-home assessments, scheduling based on member availability
- Ensure compliance with regulatory requirements while managing member interactions
What is Required (Qualifications):
- RN Diploma or Degree in Nursing (Associates or Bachelors) with a minimum of 2 years of experience in community-based services, or a Bachelor’s degree in Nursing with a Public Health Certification and at least 1 year of experience
- At least 5 years of experience beyond the degree
- Active, unrestricted RN License in the state of MN
- Strong clinical and documentation skills
- Reliable transportation and a designated home office space
How to Stand Out (Preferred Qualifications):
- Experience in home and community-based services or case management
- Experience working with seniors and individuals with disabilities
- Familiarity with diverse, low-income populations
- Knowledge of Medicare and Medical Assistance programs
- MnCHOICES Certified Assessor Training (MNCAT) or ability to obtain within 21 days of employment
