What to Expect (Job Responsibilities):
- Manage a specified caseload of members, creating individualized care plans and coordinating services
- Educate members on benefits including Medicare and Medicaid, serving as a primary contact for inquiries
- Collaborate with physicians, providers, and county financial entities to discuss care plans and member progress
- Conduct both telephonic and in-home assessments, scheduling based on member availability
- Travel within the community to meet with members, ensuring compliance with regulatory requirements

What is Required (Qualifications):
- RN Diploma or Degree in Nursing (Associates or Bachelors) or Bachelor’s degree in Social Work
- 3-5+ years of experience beyond degree, including a minimum of 2 years’ experience in community-based services
- Active, unrestricted RN OR SW License in the state of MN
- Strong clinical and documentation skills
- Reliable transportation and a designated home office area

How to Stand Out (Preferred Qualifications):
- Experience in home and community-based services/case management
- Experience working with individuals with disabilities and seniors
- Familiarity with diverse, low-income populations
- Strong knowledge of Medicare and Medical Assistance programs
- MnCHOICES Certified Assessor Training (MNCAT) or willingness to obtain within 21 days of employment
