What to Expect (Job Responsibilities):
- Manage a specified caseload of members and develop individualized care plans while coordinating the delivery of approved medical and social services
- Educate members about benefits, including Medicare and Medicaid, serving as a primary contact for addressing questions or concerns
- Collaborate with physicians, providers, and county financial entities to discuss care plans and provide updates on member progress
- Conduct telephonic and in-home assessments for members, scheduling visits based on member availability
- Ensure compliance with regulatory requirements while providing care coordination services
What is Required (Qualifications):
- RN Diploma or Degree in Nursing (Associates or Bachelors) or Bachelor’s degree in Social Work
- 3-5+ years of experience beyond degree, including a minimum of 2 years’ experience in community-based services for RN candidates or 1-2+ years for SW candidates
- Active, unrestricted RN or SW License in the state of MN
- Reliable transportation and a designated home office space
- Strong interpersonal skills and the ability to simplify medical language for non-medically trained members
How to Stand Out (Preferred Qualifications):
- Experience in home and community-based services/case management
- Familiarity with working with individuals who have disabilities and seniors
- Strong knowledge of Medicare and Medical Assistance programs
- MnCHOICES Certified Assessor Training (MNCAT) or the ability to obtain it within 21 days of employment
- Public Health Certification preferred, but not required
