Responsabilités

• Supports care management activities and coordinates care with providers, partners, and members/caregivers.
• Engages members by telephone or home visits to discuss care/service plans and next steps.
• Documents the plan for care/services and ensures continuity of care and member satisfaction.
• Communicates with care managers, practitioners, and others to facilitate member services and care plan execution.
• Supports service assessments/screenings and documents the member’s needs in compliance with regulations.
• Provides education on benefits and resources available and follows all policies and standards.
