Job description

Position OverviewThis position provides care coordination, comprehensive care management, health promotion and engagement. Natural and formal supports are actively involved. The Care Coordinator actively engages clients employing strategies to reduce unnecessary hospitalizations of clients at risk with socioeconomic issues and possibly involved in multiple systems. This position is supporting and funded by a 2-yearCertified Community Behavioral Clinic(CCBHC) Expansion Grant. Under a CCBHC, Care Coordination is defined as "deliberately organizing consumer care activities and sharing information among all of a consumer's care to achieve safer and more effective care. This means the patient's needs and preferences are known ahead of time and communicated at the right time to the right people, and that this information is used to provide safe, appropriate, and effective care to the patient."

Summary of duties:

  • Comprehensive care management and care coordination.
  • Must understand the reciprocal influences of home, school, and community to provide evidence-based interventions including crisis intervention, home visits, conflict resolution, individual and group counseling.
  • Assist in development of crisis plans, practice and rehearse them as part of the transition phase.
  • Ability to assess needs of team members when scheduling meetings or appointments with the client.
  • Increase the "natural supports" in client's and their family's integration into their community.
  • Work with clients to identify family support, peer support or other system and community resources that can assist the client with exercising their voice in the process if needed.
  • Initiate contact with clients and help access the services necessary for the continuity of client care.
  • Consult with other staff, supervisors and other client supports.
  • Be sensitive to the needs of the clients when working in rural areas where meeting with clients, and/or family members physically may be challenging. Creative in facilitating meeting via phone or through telehealth.
  • Ensure respect for the input and needs of the client in the decision-making process.
  • Acknowledge and celebrate successes and transitions.
  • Provider referral-based outreach services.
  • Be aware of the oversight of those therapeutic and case-management services, which may be provided.
  • Effective use of technology and software including but not limited to: computers, phones, email, EMR, etc.
  • Ability to work with difficult cases and vulnerable populations.
  • Be helpful, respectful, approachable and team-oriented.
  • Effective time management including but not limited to: planning ahead where possible and be punctual in person and work performed.
  • Build strong working relationships and a positive work environment.
  • Be receptive to feedback, willing to learn, and embrace continuous improvement.
  • Utilize various reports to identify and track clients who haven't followed through with referrals.
  • Coordinate with referral sources as needed to engage clients.
  • Coordinates with Community and CCBHC personnel to ensure effective client care.
  • Maintains, records and reports in accordance with agency policies and procedures.
  • Complete all documentation, reporting, and other paperwork per agency standards.
  • Attend all appropriate staff, organizational, and external meetings (as assigned).
  • Work efficiently both independently and as part of a team.
  • Ability to be flexible and manage urgent and crisis situations effectively.
  • Ability to motivate staff and develop a working team around individual cases.
  • Other duties as assigned.

Minimum Qualifications:

  • Associate’s degree preferred with at least two years of experience working in social services; or is an individual with lived experience.

in mental health, substance use disorders, or veteran's.

  • Upon hire: professional registration or certification by the Licensing Department of the State of Washington as a counselor.

Salary:$21.27 per hour.
