Care Advantage Inc. is seeking a high-energy Care Coordinator to join our South Boston, VA team. You will match caregivers with clients, coordinate schedules, and serve as the primary point of contact for families and field staff.

You’ll handle scheduling, use scheduling software, and maintain patient-focused communication. A 2+ year track record and strong organizational skills are essential, with a commitment to compassionate, flexible care.

Qualifications

• High School Diploma or GED required.
• 2+ years of relevant work experience.
• Scheduling, medical, or phone experience a plus.
• Computer literacy and comfort with scheduling systems.
• Strong organizational skills, attention to detail, and communication abilities.

Responsibilities

• Be the Matchmaker by pairing caregivers with clients based on health needs, personality fit, and preferences.
• Serve as liaison between clients, families, and field staff to maintain trust and clear communication.
• Maintain accurate, up-to-date schedules in our system and anticipate conflicts.
• Pivot quickly during emergencies or last-minute changes to keep clients cared for.
• Monitor field staff activity, attendance, and reliability to support accountability.

What You'll Do
