Job summary

Community Options for Residential in North Dakota seeks a Care Coordinator within our Behavioral Health Department. You will manage client caseloads, conduct intakes, assess needs, and develop care plans to promote client independence and overcome barriers.

You will build strong client relationships, coordinate with service providers, and make referrals to internal and external resources. You will facilitate client team meetings and ensure follow-up to achieve goals.

Qualifications

• An associate degree in a related field is required, OR 1 year of direct services experience.
• Requires valid driver’s license and reliable transportation.
• Ability to travel as needed to serve clients and attend meetings.
• Completion of state care coordination training may be required.

Responsibilities

• Conduct client intakes and assess needs to inform care planning.
• Develop and implement care plans to promote client independence.
• Coordinate services with internal/external providers and resources.
• Facilitate client team meetings and ensure timely follow-up.
