Ennoble Care is seeking a Care Coordinator, Embedded, to work full-time in our client’s senior living facility in Westwood, NJ. You will coordinate patient care plans, communicate with clinicians, and support caregivers and families to ensure high-quality care.

The role requires exceptional organizational skills, compassion, and proficiency with Outlook and MS Office. You will engage daily with patients, families, and healthcare teams to maintain care continuity in a fast-paced environment.

Qualifications

• Must be comfortable with speaking on the phone for large amounts of the day.
• Compassionate and empathetic toward patients with exceptional customer service.
• Ability to take accurate notes and document tasks in a timely manner.
• Multitask between different patients and workstreams while remaining organized.

Responsibilities

This position is responsible for ensuring Ennoble Care is providing high quality care services. They work with clinicians, staff and patients to reach healthcare goals and keep the lines of communication open. As a Care Coordinator, you should be compassionate, experienced, and highly organized. In this role, you will play an important part in our ability to provide exceptional care by managing the individual care providers, including scheduling and providing support for the caregivers and families.
