Job summary

Ennoble Care is seeking an Embedded Care Coordinator to join our on-site team at partner long-term care facilities around the Reston and Warrenton area in Virginia. You will coordinate care for residents, collaborate with clinicians, and engage families to ensure continuity of care in a family-practice style model.

The role is full-time, Monday–Friday, with daytime hours and no weekends or on-call, based in Northern Virginia, and requires active licensure and at least one year of patient-facing

Qualifications

• Legally authorized to work in the United States
• Active, unrestricted clinical certification/licensure (PCT, CNA, MA/CMA, LPN, RN, or comparable)
• Minimum 1 year of patient-facing experience in a long-term care
• Proficient in Microsoft Teams, email, phone and in-person communication
• Positive relationships with residents, families, providers, and staff
• Strong organizational and time-management skills
• Strong healthcare terminology and care coordination knowledge
• Proficiency with Microsoft 365 (Outlook, Word, Excel)
• Full-time, on-site, 8:00 a.m.–4:30 p.m. EST, Reston, VA area, Warrenton vicinity
• Valid driver's license, reliable transportation, proof of auto insurance
• Ability to pass a background check

Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Manassas, Virginia area. This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support.

WHAT 'EMBEDDED' MEANS AT ENNOBLE CARE

This is not a remote or call-center role.

As an Embedded Care Coordinator, you are physically on-site in one or more partnered long-term care facilities, working alongside providers and facility staff. You build consistent relationships with residents and families and become a trusted, familiar presence within the facility -- supporting care the way a traditional family practice would, but in the long-term care setting. To support a dedicated group of patients. The work is very much family-practice focused, including a strong emphasis on remote patient monitoring (RPM) and proactive care coordination.

REQUIREMENTS

• Legally authorized to work in the United States of America
• Active, unrestricted clinical certification or licensure, such as:
  • Patient Care Technician (PCT)
  • Certified Nursing Assistant (CNA)
  • Medical Assistant (MA) / Certified Medical Assistant (CMA)
  • Licensed Practical Nurse (LPN)
  • Registered Nurse (RN)
  • Or comparable credential
• Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
• Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person
• Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting
• Well-developed organizational and time-management skills, paired with an optimistic approach and initiative-driven mindset
• Strong working knowledge of healthcare terminology and care coordination principles
• Proficiency with Microsoft 365, including Outlook, Word, and Excel
• Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, (or thereabouts, contingent on facility hours) in an on-site role, based in the Reston, VA area, with a primary focus on the Warrenton vicinity.
