Job summary

ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to manage care coordination for adolescents, including orientation, transportation, doctor visits, and scheduling. Under supervision of the Program Director, you will ensure adherence to standards and coordinate activities to support clients and families.

The role requires strong communication, teamwork, and meticulous documentation in an EMR system, with a focus on safety and confidentiality in daily operations.

Qualifications

  • Minimum high school diploma; Bachelor’s degree preferred.
  • Two years of driving experience required.
  • One year minimum experience working with kids in a therapeutic setting.
  • Experience with junior high or high school students preferred.
  • Ability to communicate with people from diverse educational and cultural backgrounds.
  • Ability to cope with stressful situations with tact and professionalism.
  • Strong written communication and organizational skills.

Responsibilities

  • Coordinate the care and services for ROWI clients under supervision.
  • Arrange orientation, transportation, doctor visits, and schedules.
  • Ensure programs meet high standards and clients receive quality care.
  • Supervise clients and ensure attendance at designated groups.
  • Conduct client orientation and necessary assessments.
  • Maintain EMR documentation and confidentiality.

Position Summary:

Under the supervision of the Program Director the Care Coordinator is responsible for anything related to the coordination of care for all ROWI clients. The Care Coordinator will coordinate individualized service including orientation, transportation, doctor visits, and schedule.

Duties and Responsibilities:
  • Ensures programs and services are implemented at the highest standards and clients receive the highest level of care.
  • Responsible for client supervision and ensuring they attend designated groups throughout programming.
  • Facilitate assigned groups and activities as designated by the Facility Manager.
  • Conduct client orientation for all admits on day of admission or assign to another staff if not available. Including UA screening, PHQ9 and other assessments as directed.
  • Ensure compliance with Joint Commission Standards.
  • Document shift reports and share pertinent information with the team.
  • Coordinate the transportation schedule for all clients and drive clients as needed.
  • Document all transportation and group activity in EMR.
  • Administer and submit documentation in EMR for client Urine Analysis.
  • Assist with facility cleanliness as needed.
  • Responsible for coordinating client lunch daily.
  • Serve as a role model to treatment staff, clients, employees, and guests through a positive, caring, and professional attitude.
  • Maintain the confidential nature of clients and related activities.
  • Comply with the dress code.
  • Complete tasks in a timely and accurate manner.
  • Attend designated staff and in‑service meetings.
  • Any additional duties as assigned by the management team.
  • Ability to maintain clear boundaries with clients and staff, set limits and manage clients when in need of direct supervision. Maintains appropriate and professional interactions with clients and staff.
  • All documentation in EMR must be submitted within 24 hours.
