Job summary

ROWI Temecula is seeking a Care Coordinator to coordinate care for adolescents and families in a structured program. You will handle orientation, transportation, doctor visits, and scheduling, working under the Program Director to deliver consistent, high-quality care.

Responsibilities include supervising clients in groups, arranging activities, conducting admissions orientation with PHQ9 assessments, and documenting in EMR.

Qualifications

  • High school diploma required; bachelor’s preferred.
  • Two years driving experience required.
  • One year experience with children in therapy setting.
  • Experience with middle/high school students preferred.
  • Able to communicate with diverse backgrounds.

Responsibilities

  • Coordinate care for ROWI clients: orientation, transport, doctor visits, scheduling.
  • Ensure highest care standards; supervise clients in groups.
  • Facilitate groups/activities; conduct client orientation.
  • Perform UA screening, PHQ9 as directed; ensure Joint Commission compliance.
  • Document shift reports; coordinate transportation; EMR entries.
  • Maintain facility cleanliness; coordinate client meals; professional conduct.
  • Maintain confidentiality; adhere to dress code; timely task completion.
  • Attend staff meetings and take on additional duties as needed.

Position Summary

Under the supervision of the Program Director, the Care Coordinator is responsible for coordinating care for all ROWI clients. Responsibilities include individualized service coordination such as orientation, transportation, doctor visits, and scheduling.

Duties and Responsibilities
  • Ensures programs and services are implemented at the highest standards and clients receive the highest level of care.
  • Responsible for client supervision and ensuring they attend designated groups throughout programming.
  • Facilitate assigned groups and activities as designated by the Facility Manager.
  • Conduct client orientation for all admits on day of admission or assign to another staff if not available. Including UA screening, PHQ9 and other assessments as directed.
  • Ensure compliance with Joint Commission Standards.
  • Document shift reports and share pertinent information with the team.
  • Coordinate the transportation schedule for all clients and drive clients as needed.
  • Document all transportation and group activity in EMR.
  • Administer and submit documentation in EMR for client Urine Analysis.
  • Assist with facility cleanliness as needed.
  • Responsible for coordinating client lunch daily.
  • Serve as a role model to treatment staff, clients, employees, and guests through a positive, caring, and professional attitude.
  • Maintain the confidential nature of clients and related activities.
  • Comply with the dress code.
  • Complete tasks in a timely and accurate manner.
  • Attend designated staff and in-service meetings.
  • Any additional duties as assigned by the management team.
  • Ability to maintain clear boundaries with clients and staff, set limits and manage clients when in need of direct supervision. Maintains appropriate and professional interactions with clients and staff.
  • All documentation in EMR must be submitted within 24 hours.

Education and Experience Required
  • The Care Coordinator must have a minimum of a high school diploma; a bachelor’s degree is preferred.
  • Minimum of two (2) years driving experience required.
  • Minimum of 1 year working with children in a therapeutic setting.
  • Experience with junior high or high school students is preferred.
  • Must be able to communicate with people from all educational and cultural backgrounds.
  • Must cope constructively with stressful situations.
  • Must demonstrate tact, initiative, resourcefulness, professional manner, enthusiasm, and non‑judgmental attitude.
  • Must have strong written communication skills.
