Suffolk Family Carers in Suffolk is seeking a proactive Care Coordinator to join the Care Management Service. This contract role supports individuals and families, coordinating support from multidisciplinary services to reduce duplication and improve timely, person-centred care.

The role is field-based, 37 hours per week across Ipswich and surrounding areas (Ipswich West, Ipswich East & South Rural, Bury St. Edmunds, Central Suffolk).

Qualifications

• Coordinating referrals into the Care Management Service to ensure timely support.
• Working with health, social care, voluntary and community partners for joined-up, person-centred care planning.
• Maintaining records and signposting to resources while liaising with patients, families and carers.

Responsibilities

• Coordinating referrals into the Care Management Service and helping ensure people receive the right support from the right service at the right time.
• Working with health, social care, voluntary and community partners to ensure joined-up, efficient and person-centred care planning.
• Maintaining accurate records, signposting to appropriate resources, supporting multidisciplinary meetings and liaising with patients, families and carers by phone, in clinics or during home visits as required.
