Job summary

Qualifications

• Licensed Social Worker (LSW) or Registered Nurse (RN) licensed to practice in Minnesota.
• Experience with assessments and knowledge of community resources.
• Valid driver's license.
• Background check and motor vehicle record required.
• Strong written and verbal communication skills.

Responsibilities

• Conduct initial and annual comprehensive assessments to enrollees.
• Create an individualized Plan of Care and coordinate provision of services.
• Provide culturally appropriate, person-centered care.
• Promote enrollee knowledge, resources and self-advocacy; ensure accessibility and accountability.
