Responsibilities

Coordinate holistic care by collaborating with physicians, families, and caregivers to craft individualized care plans.
Monitor daily services, review progress toward goals, and retrain staff when updates are needed.
Lead client advocacy by accompanying clients to appointments and relaying updates to the team.
Train caregivers and assist HR with onboarding and field staff education.
Build caregiver schedules balancing continuity of care with staff preferences.
Document assessments and outcomes in EMR/CRM and generate quality reports.
Resolve urgent issues with calm, speed, and compassion on 24/7 on‑call rotation.
