Job summary

Cowlitz Family Health Center seeks a Behavioral Health Consultant to join a primary care team. You will deliver brief, evidence-based behavioral health services, coordinate care, and support patients with psychoeducation and engagement strategies.

The role emphasizes collaboration with providers, attention to safety, and continuity of care in a fast-paced community setting. Washington licensure is required and some duties may occur via telehealth.

Qualifications

Wage Range: $73,171.00 - $78,797.16 Annually

Schedule: Monday - Friday 8:00am - 5:00pm

Job Summary

The Behavioral Health Consultant (BHC) is an integral member of the primary care treatment team, providing integrated behavioral health services to patients within the primary care setting. The BHC works collaboratively with primary care providers and other members of the care team to identify, assess, and address behavioral health, substance use, and psychosocial concerns that may impact a patient’s overall health and well-being.

This position provides brief, evidence-based behavioral health interventions, psychoeducation, care coordination, clinical assessment, and ongoing patient support designed to help patients achieve meaningful improvements in their health and quality of life. The BHC may work with patients in person, by telephone, or through video visits, utilizing warm handoffs and other engagement strategies to connect patients with care when they need it most.

The ideal candidate is flexible, organized, persistent, compassionate, and highly collaborative, with the ability to prioritize pressing patient needs while adapting to a dynamic primary care environment. The BHC partners closely with providers and community resources to support treatment planning, referrals, recovery, relapse prevention, medication adherence, and continued engagement in care.

Requirements
  • Current Washington state licensure as a Licensed Marriage and Family Therapist, Licensed Mental Health Counselor or Licensed Clinical Social Worker.
  • Dual SUD certification preferred.
DESIRED EDUCATION, SKILLS AND EXPERIENCE
  • Master’s or Doctorate degree in social work, marriage and family therapy, psychology, behavioral health or related field.
  • Demonstrated ability to collaborate effectively in a team setting.
  • Ability to maintain effective and professional relationships with clients and other members of the care team.
  • Strong communication skills.
  • Ability to manage a high volume of patients on a daily basis and the pace of primary care.
  • Ability to assess quickly and make decisions.
  • Competent in assessing suicide lethality in patients and following CFHC protocol treatment and follow-up procedures.
  • Ability to work well under pressure, effectively engage clients and deal with confrontation.
  • Ability to work with clients by telephone or in person.
  • Working knowledge of evidence‑based video psychosocial treatments for common mental disorders.
  • Familiarity with brief evidence‑based counseling techniques, (e.g., Motivational Interviewing, Solution Focused Therapy Cognitive Behavioral Therapy, Trauma informed therapy, Dialectical Behavior Therapy).
  • Basic knowledge of psychopharmacology for common mental disorders.
  • Experience working with underserved, transient populations.
  • Experience working with patients who have co‑occurring mental health, substance use, and physical health problems.
  • Experience working with safety net providers within the community and knowledge of community resources.
  • Working knowledge of SSI and other disability program requirements.
  • Ability to partner with community partners
  • Working knowledge of the publicly funded specialty mental health and substance use treatment system’s structure and regulations.
