Qualifications

• Must be Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW).
• 2+ years of professional experience as an LSW or LCSW.
• 2+ years of Case Management, Community Health, or Care Coordination experience.
• Must be able to travel locally (valid driver's license and auto insurance required).

Responsibilities

• Coordinates care for participants and provides educational information regarding medication and disease states.
• Performs assessments and data analysis of claims to determine appropriate care needed.
• Develops/implements appropriate care plans based on assessment findings.
• Identifies barriers and potential risks.
• Provides needed social support and resources needed for care including benefits/community services.
