AMN Healthcare is seeking a Care Coordinator/Case Manager for an RN role in Carmichael, CA. This mid-shift assignment supports hospital discharge planning, coordination across providers, and utilization review to ensure safe, cost-effective care.

You will work 12-hour shifts 1200-2030, every other weekend, for a 13-week assignment, with Cerner as the system. California RN license and BLS are required.

• 3 years hospital experience with acute level care coordination
• Experience with discharge planning and utilization review
• Prior CA Dignity/Commonspirit acute care experience in specialty preferred

PPOSITION SUMMARY – RN Case Manager/Care Coordination - Mid-shift

POSITION DUTIES

Coordinate patient care across providers, oversee utilization review to ensure appropriate and cost-effective services, and facilitate timely, safe discharge planning by arranging post-acute resources and support.

Minimum Required Qualifications

• CA RN license
• BLS
• 3 years hospital experience and acute level care coordination experience
• Skilled experience with discharge planning and utilization review
• Prior CA Dignity/Commonspirit acute care experience in specialty

PREFERRED QUALIFICATIONS – Minimal Time Off Requests
