Visiting Nurse Health System in Atlanta, GA is seeking a Care Coordinator, BSW to support our Fulton and Dekalb county members. You will develop individualized care plans, coordinate services with the care team, and monitor outcomes to ensure professional, cost-effective care.

The role requires a Bachelor’s degree in Social Work or related field and at least two years of case management experience, including Medicaid/Medicare coordination; strong communication and organization skills are

Qualifications

Bachelor’s degree in Social Work, Sociology, Psychology, or related field.
Two years of case management experience in a health care setting.
Two years of experience with Medicare, Medicaid and other funding sources.
Reliable transportation including a valid driver's license and proof of vehicle insurance.

Responsabilités

Coordination of CCSP and non-CCSP community-based services with the RN care coordinator, client, and family members.
Coordinate Medicaid application team to assure CCSP accessibility for eligible clients.
Coordinate with lead agency or DHR to ensure CCSP components meet client needs.
Develop and implement care plans; monitor and document progress and outcomes.
Document all care management activity and maintain confidential records.
Ensure compliance with DHR/DHS standards and prompt payment/verification requirements.
