We are looking for a dynamic and compassionate Spanish Speaking Care Coordinator to work with us. In this role, you will guide program enrollees and their caretakers (legal guardians) through the healthcare system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes.

Location: Bronx, NY 10453
Hours: 35 hours, Non-Exempt
Salary Range: $44K-$48K annually
Major Duties of Bilingual Spanish Care Coordinators
- Obtain required Care Management enrollment consents from individuals or legal guardians.
- Complete initial and ongoing needs assessments (Child and Adolescent Needs and Strengths; CANS) to determine appropriate care management levels.
- Manage the patient’s Individualized Plan of Care, including:
- Coordinating service provision based on acuity level.
- Supporting adherence to treatment recommendations.
- Monitoring and evaluating needs related to prevention, wellness, medical, mental health, care transitions, and social services.
Meet client contact requirements with face-to-face visits as mandated by acuity level.
Accurately document in the designated Care Management Portal (MAPP) and EHRs.
Advocate for clients within the agency and with external providers.
Promote wellness by linking enrollees with resources and services.
Educate children and caregivers on managing chronic conditions and preventive care.
Assist clients in obtaining public benefits such as Medicaid, SNAP, housing, and legal services.
Communicate effectively considering language, literacy, and cultural preferences.
Conduct care planning meetings and coordinate comprehensive care as part of an interdisciplinary team.
Identify and manage community resource referrals, access, engagement, and follow-up.
Participate in discharge planning during hospital admissions to ensure post-discharge services are in place.
Attend ongoing training to enhance skills.
Ensure periodic evaluations and follow-up for dental, vision, and hearing care per Medicaid EPSDT guidelines.
Perform other duties as needed.
Qualifications
Education and credential requirements depend on children’s acuity level, including:
- Bachelor’s degree required.
- Experience as a Case Manager, Case Planner, or Care Coordinator in social services.
- Experience with Child Welfare and/or Foster Care is advantageous.
- Bilingual Spanish required.
Required Experience
- Experience serving children and families in child welfare, developmental disabilities, mental health, healthcare, or preventive services.
- Knowledge of high-acuity needs, with additional experience or training.
- Experience in coordinating care for children with medical fragility.
- Proven ability to provide service coordination, referrals, and community linkages.
