Job summary

Magellan Health is seeking a Care Coordinator for Eddy, Lea, and Chaves counties to manage community-based and facility-based care coordination for members with behavioral health needs.

The role covers assessment, care planning, implementation, and monitoring to ensure quality outcomes. Requires 3–5 years in social work or healthcare, with strong communication and collaboration skills.

Qualifications

• 3-5 years' experience in Social Work, Nursing, or Healthcare-related field required.
• Experience in utilization management, quality assurance, home or facility care, community health, long term care or occupational health required.
• Knowledge of cost-effective coordination of care and interpretation of data.
• Ability to make decisions requiring significant analysis and investigation with original thinking.
• Ability to determine appropriate courses of action in complex situations.

Responsibilities

• Provide care coordination to members with behavioral health conditions identified and assessed as requiring intensive interventions and oversight including multiple clinical, social and community resources.
• Conduct in-depth health risk assessment and/or comprehensive needs assessment including psycho-social, physical, medical, behavioral, environmental, and financial parameters.
• Communicate and develop the care plan and serve as point of contact to ensure services are rendered appropriately.
• Implement, coordinate, and monitor strategies for members and families to improve health and quality of life outcomes.
• Develop, document and implement plans providing resources to address social, physical, mental, emotional, spiritual and supportive needs.
• Advocate for member's care needs by identifying and addressing gaps in care.
• Monitor the plan of care to evaluate effectiveness and measure intervention outcomes.
• Collaborate with interdisciplinary Care Plan team to address care issues and disease processes.
