Job summary

Texas Children’s Hospital in Houston is seeking a Care Coordinator Assistant to provide non-clinical administrative support to the Care Coordination process, including timely discharge planning and utilization management.

This role requires a high school diploma or GED, some college coursework, and about two years of hospital care coordination or related experience. You will communicate with internal and external customers, maintain data integrity, and support a collaborative care team.

Qualifications

  • HS Diploma or GED is required.
  • Some college coursework preferred to support care coordination duties.
  • 2 years of experience in hospital care coordination/case management or related field required.

Responsibilities

  • Provides department-related and other appropriate communications to internal and external customers.
  • Provides assistance for utilization management processes to maintain compliance with federal, state and managed care guidelines.
  • Provides assistance for transition/discharge planning and care coordination processes to transition plans and enhance patient/family satisfaction.
  • Maintains data integrity.
  • Provides professional, efficient and courteous service to the customer while maintaining integrity in dealing with issues of a confidential nature.
  • Works effectively with peers, nursing and medical staff and maintains a positive attitude. Responds to requests personally, tactfully, and courteously.
  • Obtains information required for response whenever possible and communicates pertinent information to staff, customers, and providers. Relays messages to appropriate individuals efficiently and in a timely manner.
  • Promotes a positive work environment, and contributes to the overall team effort.
