Job summary

Lutheran Social Services of Wisconsin and Upper Michigan (LSS) seeks a full-time Care Coordinator for the Birth to Three Program. This federally mandated Early Intervention program supports children aged 0–3 with developmental delays or disabilities and their families.

The role focuses on strengthening families, enhancing child development, and coordinating services using a family-centered, trauma-informed approach.

Qualifications

  • Bachelor’s degree in Social Work, Child Development, or related field.
  • One year of supervised experience working with families with special needs preferred.

Responsibilities

  • Conduct functional assessments and reassessments.
  • Develop and implement individualized, family-centered service plans.
  • Coordinate and facilitate services aligned with care plans.
  • Support families through education, advocacy, and collaboration.
  • Maintain accurate and timely documentation in compliance with regulations.

  • Collaborate with internal teams, supervisors, and community providers.
  • Manage a flexible schedule based on client needs, including occasional evenings or weekends.

Job description

Care Coordinator – Birth to Three Program

Lutheran Social Services of Wisconsin and Upper Michigan (LSS) is seeking a full-time Care Coordinator to join our Birth to Three Program, a federally mandated Early Intervention program supporting children ages 0–3 with developmental delays or disabilities and their families.

This role focuses on strengthening families, enhancing child development, and coordinating services using a family-centered, trauma-informed approach. This is a full time, benefit eligible role working Mondays-Fridays from 8am-4/4:30pm. After a training period, this role will have remote flexibility when not at home visits with clients, but the team does meet at the Waukesha office every other Tuesday for mandatory staff meetings.

What You’ll Do

As a Care Coordinator, you will partner closely with families, service providers, and community resources to support young children’s development.

Key responsibilities include:

  • Conducting functional assessments and reassessments

Qualifications

Education & Experience

  • Bachelor’s degree in Social Work, Child Development, or related human services field required
  • One year of supervised experience working with families with special needs preferred

  • Valid driver’s license and reliable transportation
  • Ability to meet LSS driving and insurance requirements
  • Strong documentation, communication, and organizational skills
  • Comfortable working in home, office, and community-based settings
  • Ability to respond appropriately in crisis situations
  • Community-based role with daily travel
  • Moderate physical activity, including bending, kneeling, stair climbing
  • Travel may include occasional overnight stays
  • Exposure to various environments while serving families in their homes and communities
