Job summary

Pro Care by Abound Health is seeking a compassionate and organized Care Coordinator in Southfield, MI. Reporting to the Program Director, you’ll help clients live more independently and connect them with needed support.

You will supervise direct care staff, coordinate services, and maintain records in OnTarget while upholding IPOS and waiver requirements. This role offers a comprehensive benefits package and a 401(k) plan after 12 months.

Qualifications

• High school diploma or GED required.
• Experience in care coordination, case management, social services, or healthcare.
• Strong communication, organization, and problem-solving skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Knowledge of IPOS and Michigan Waiver Services preferred.
• Proficiency in Microsoft Office; OnTarget experience a plus.
• Valid driver’s license and reliable transportation.

Base pay is determined by factors such as work location, experience, job‑related skills, and relevant education or training. In addition to base pay, total compensation may include various other forms of pay. We offer a comprehensive benefits package, including competitive health, dental, and wellness benefits, which begin on your 60th day of employment.
