The Care Coordinator at Cumberland River Behavioral Health is a key member of a multidisciplinary team serving adults, children, and families in Kentucky. You will help clients access medical, psychiatric, social, and other support services, coordinate referrals, and respond to crises.
Qualifications include a Bachelor’s, Master’s, or Associate degree in relevant fields and background checks. The work schedule is structured in 7.5-hour blocks totaling 37.5 hours per week, with flexibility as
A Bachelor’s degree in Psychology, Sociology, Human Services or Nursing from an accredited school; or a Master’s Degree in Human Services field from an accredited school with or without licensure; or Associate’s Degree in Nursing from an accredited school. Experience working in healthcare customer service preferred. Submit to and pass a criminal background check and drug screening.
Work hours are structured in 7.5 hour blocks per day, totaling 37.5 hours per week. Schedules may be adjusted to accommodate irregular or flexible hours as required by agency needs.
This position reports directly to the Team Leader if located in an outpatient facility and/or Program Director, if in a residential program.
The Care Coordinator is a member of a multi-disciplinary team responsible for providing services to adults, children and families presenting for services. The Care Coordinator is to assist helping the person gain access to medical, psychiatric, social, educational and other support services.
