The Care Coordinator at Cumberland River Behavioral Health is a member of a multidisciplinary team helping adults, children and families access medical, psychiatric, social, educational and other support services. You will promote wellness and recovery while removing barriers to care and coordinating referrals.
This role involves 37.5 hours per week, with shifts in 7.5 hour blocks and potential schedule adjustments. You will report to the Team Leader or Program Director depending on setting.
A Bachelor’s degree in Psychology, Sociology, Human Services or Nursing from an accredited school; or a Master’s Degree in Human Services field from an accredited school with or without licensure; or Associate’s Degree in Nursing from an accredited school. Experience working in healthcare customer service preferred. Submit to and pass a criminal background check and drug screening.
Work hours are structured in 7.5 hour blocks per day, totaling 37.5 hours per week. Schedules may be adjusted to accommodate irregular or flexible hours as required by agency needs.
This position reports directly to the Team Leader if located in an outpatient facility and/or Program Director, if in a residential program.
