Job summary

Communicare, Inc. in Bardstown, KY is seeking a Care Coordinator to oversee coordination of all aspects of care for clients in a Certified Community Behavioral Health Clinic (CCBHC).

The role builds effective partnerships with clients, their families, community resources and physicians to deliver coordinated, high‑quality, client‑centered care. The ideal candidate may be a Licensed Practical Nurse or hold a Bachelor's degree in a human service field, with at least five years of care coordination

Qualifications

  • Licensed Practical Nurse or Bachelor's degree in a human service field with at least 5 years of care coordination experience.
  • RN or PA licensed to practice in Kentucky.
  • Experience navigating local medical and social support systems preferred.
  • Experience with health systems, data reports, and care coordination preferred.

Responsibilities

  • Promotes client-centered health care concepts with clients and staff.
  • Assists with development and coordination of the Plan of Care with the multidisciplinary treatment team.
  • Develops short and long term strategies and collects data to measure outcomes.
  • Collaborates with providers and staff to deliver quality care for best client outcomes.

POSITION SUMMARY

P rovides coordination of all aspects of care for clients in Communicare’s Certified Community Behavioral Health Clinic (CCBHC). Coordinates effective partnerships with clients, their caregivers/families, community resources and their physician(s). Facilitates a service model of shared goals within and across settings to achieve coordinated high-quality care that is client and family centered. Experience and knowledge in behavioral health and primary care programming is strongly preferred.


QUALIFICATIONS

Licensed Practical Nurse or a Bachelor’s degree in a human Service Field with at least 5 years of care coordination experience, Registered Nurse, or Physician Assistant licensed to practice in Kentucky. Experience with navigation of local medical and social support systems is preferred. Previous experience with health systems, data reports, and care coordination is preferred. Demonstrated experience with diverse adults receiving mental health treatment, including the delivery of treatment to individuals with serious mental illness.

Ability to effectively work with and through physicians, subordinates, customers, and other personnel in a cooperative, collaborative manner in order to improve clinical quality outcomes.


PHYSICAL AND ENVIORMENTAL DEMANDS

Visual acuity necessary for updating files and completing documents. Requires frequent stooping, kneeling and crouching to file records. Hearing and talking necessary to answer phone and talk with employees and other individuals. Work is light with walking, standing, and sitting. Manual dexterity is required to perform word processing and other clerical duties.


JOB SPECIFIC FUNCTIONS


  • Promotes and reinforces client centered health care concepts with clients and staff.

  • Assists with the development, revision, and coordination of the Plan of Care through collaboration with the multidisciplinary treatment team to meet the clients’ needs effectively and efficiently.

  • Develops short term and long term strategies in the development of expected client outcomes; collects data through client tracking in order to measure outcomes.

  • Works collaboratively with provider(s) and other staff to ensure the delivery of quality care to clients to ensure best client outcome.

  • Assesses, plans, implements, coordinates, and evaluates the effectiveness of the client programs.

  • Collects data through client tracking in order to facilitate client outcome data collection and analysis.

  • Serves as a resource contact and information/education source to clients, families, providers, and/or staff.

  • Assists the development, revision, and implementation of client programs and/or marketing programs, materials, and resources in specialty area to meet the specific needs of the client population.

  • Coordinates and directs clients and/or families to appropriate resource utilization.

  • Works with providers, staff from other agency locations, third party payers, families, community resources, etc. to facilitate care of the client throughout the continuum of care.

  • Identifies system/organizational processes which may affect effective utilization of resources, timely scheduling of tests, appropriate level of care being given, etc., and collaborates with team members to improve upon the processes.

  • Responsible for coordinating and leading client support groups and/or shared medical appointments.

  • Identifies practice and/or program improvement opportunities to assist with securing the highest level of payment/reimbursement.

  • Maintains knowledge and satisfactorily complies with regulatory and third party payers’ procedures, notification requirements and criteria, documentation required in the electronic health record, etc.

  • Participates in quality improvement reviews.
