Home Care Evolution is seeking a Care Coordinator to collaborate with caregivers, physicians, patients, and families to determine medical needs and oversee employment to ensure high-quality, individualized care. The role emphasizes compassion, reliability, and knowledge of homecare practices.

The ideal candidate will coordinate care plans, train staff, and manage 20-40 clients while maintaining strong communication and organizational skills in a fast-paced environment.

Qualifications

• Care coordination experience or similar role preferred.
• Medical assistant certification preferred.
• Strong problem-solving and organizational skills.
• Ability to manage multiple tasks and prioritize effectively.
• Ability to work in fast-paced environments and make quick decisions.

Responsibilities

• Collaborate with physicians, patients, families, and healthcare staff.
• Review Care Plan to ensure it is being followed; update/train if needed.
• Develop individualized care plans.
• Accompany clients to Doctor/Hospital visits.
• Create goals and monitor progress toward them.
• Recruit and train staff.
• Create schedules.
• Manage 20-40 clients.
• Handle after-hours phone duties.

Qualifications

• Previous experience as a Care Coordinator or in a similar position is preferred


• Certification as a medical assistant is preferred


• Strong problem-solving and organizational skills


• Ability to manage multiple projects or tasks and prioritize appropriately


• Ability to work in fast-paced situations and make sound decisions quickly


• Excellent interpersonal skills and high level of compassion


• Strong verbal and written communication skills
