This role mixes office work and field visits, 50% in the office and 50% with clients. Responsibilities include coordinating caregivers, assessing clients, and maintaining accurate records while upholding standards of care.

Qualifications

Responsibilities

• Coordinate and schedule caregivers to match client needs, preferences, and care plans
• Serve as the primary point of contact for clients, families, and caregivers regarding care services
• Conduct client assessments and develop individualized care plans with the care team
• Monitor caregiver performance and ensure compliance with home care standards
• Manage caregiver scheduling, shift changes, and on‑call coverage to maintain continuity of care
• Maintain accurate client and caregiver records in compliance with regulatory requirements
• Assist with caregiver onboarding, training, and ongoing support
• Respond promptly to client concerns and resolve service issues with professionalism and empathy
